Leveraging technology & data analytics to improve case management in population health programs

As healthcare continues to evolve, the integration of technology in case management has become increasingly important.  Fear and hesitancy to adopt new and advanced communication and analytics technology may be limiting today’s professional case manager. Often, it is unclear how these tools may impact their day-to-day work or further build a trusting relationship with their clients. Today, technology facilitates many daily tasks, but how will more advanced technologies (like AI) facilitate and enable the broader goals of population health?

This session is a panel discussion with leading industry experts discussing leveraging technology & data analytics to improve case management in population health programs

This is a collaborative production between the Commission for Case Manager Certification and the Population Health Alliance.

  • List two ways that AI can support decision-making for population health.
  • Demonstrate the application of data analytics to improve care coordination and client outcomes.
  • Define strategies to integrate social determinants of health into care planning and enhanced client engagement to improve population health.
